LUMS card to reflect external request procedure

on ELIXIR homepage we will have a button to request LUMS access if none of the IdPs fit.

can you please update the external howto for requesting a LUMS account to reflect the “external” process? Eg find a manager at UL, fill and sign form, approve ToS, etc?
